Welcome to Dodge Center Family Dentistry. To ensure your visit is as smooth as possible, please review our office protocols and policies below.
We are proud to be in-network providers for:
Yes. We see many out-of-network patients and will submit claims to your insurance as a courtesy. For these visits, the full fee for services rendered is due on the day of your appointment. We then submit the procedural codes to your insurance company, and they will send the reimbursement payment directly to you.
We accept all major credit cards, debit cards, cash, and checks. We also gladly accept FSA (Flexible Spending Account) and HSA (Health Savings Account) funds.
Pre-payment Discount: We apply a 5% discount for treatments paid in full on or before the day of your appointment.
Payment Plans: We do offer flexible payment options. Please speak with our front desk team to inquire about the plans available for your treatment.
All intake forms are handled digitally through our secure patient portal. Once you schedule your appointment, you will receive a link via text or email. These forms must be completed and submitted online prior to your visit.
We want to ensure there are no financial surprises during your visit. Submitting your insurance details at least 5 business days prior to your appointment gives our team the necessary time to verify your specific benefits and provide you with an accurate estimate of your co-pay and out-of-pocket costs.
If you are scheduled with less than 5 days' notice (due to a last-minute opening or an emergency), please be aware that we may not be able to accurately estimate your insurance co-pay. In these instances, you will be responsible for any balance remaining on the account once your insurance company has processed the claim.
Adults: Your first visit is a Comprehensive Oral Evaluation. This includes a full series of digital X-rays (FMX) to establish a complete baseline of your oral health.
Children: This includes a comprehensive exam, a professional cleaning (prophy), and X-rays as determined necessary by the doctor based on your child's age and clinical needs.
We value your time and reserve a specific block of time exclusively for you. If you need to reschedule, we require at least 48 hours' notice.
Taking your blood pressure is the standard of care in modern dentistry. Your oral health and systemic health are deeply connected. Certain medications and anesthetics used in a dental setting can be contraindicated if your blood pressure is outside a safe range.
This screening is a non-negotiable safety protocol. If a patient declines this screening at the chair, we cannot proceed with the appointment. If your blood pressure is outside of a safe range, medical clearance from your physician will be required before moving forward.
Please fill out and sign our Release of Records form within your digital intake packet. This allows us to contact your previous office and ensure your relevant history and X-rays are sent to us before you arrive for your comprehensive exam.
